Summary Care Record
What is a SCR (Summary Care Record)?
A summary care record is a national database that contains an electronic record which holds information about your current medication, allergies and details of previous adverse reactions to medication. Only authorised healthcare professionals involved in the patient’s direct care have access to this information.
What are the benefits of having a SCR?

Do I have to have one?
No – you can opt out by completing the following form and returning it to your registered surgery.

If you do choose to have a SCR – you do not need to do anything as the record is automatically created when you register at a GP practice in England.
Will your care be affected if you opt out?
No – deciding to ‘opt out’ will not affect how information is used to support your care and treatment.
