Register with our practice

We welcome new patients who live within our practice area to register with us.

Anyone in England can register with a GP surgery. It’s free to register. You do not need proof of address or immigration status, ID or an NHS number.

GP surgeries are usually the first contact if you have a health problem. They can treat many conditions and give health advice. They can also refer you to other NHS services. You do not need proof of ID to register with a GP, but it might help if you have one or more of the following:

Passport | Birth certificate | HC2 certificate | Rough sleepers’ identity badge | Hostel or Accommodation registration |Mail forwarding letter

If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address.

Please note: You will be asked to upload the ID proof in case you request online access to medical records.

What are the acceptable examples of Photo ID and Proof of Address?

Acceptable photo ID:

  • Current passport or identity card
  • Resident permit issued by Home office
  • Current UK photocard driving licence
  • HM Forces identity card
  • Police photographic ID card
  • University or college photo smartcard
  • NHS smart card

Acceptable proof of address:

  • Recent utility bill (within 3 months of the current date)
  • Local authority tax bill ( valid for current year)
  • Bank or building society statement ( within 3 months of a current date)
  • Current tenancy agreement
  • Court order (within 12 months of the current date)
  • UCAS letter (within 12 months of the current date)

How do I register my baby?

You need to register your new baby at the Practice as soon as possible.

Parents or guardians can now register a baby at the practice by completing our online registration form. Should this not be accessible, please visit the practice and we will provide a paper form to complete.  Older babies and children should also register using our Join the Practice Request online form.

Mother and Baby Checks

Once your baby is registered, the Practice will send you an appointment for a mother and baby post-natal check. This takes place at around 6 weeks.

This is a 20 minute appointment; 10 minutes for the baby’s check and 10 minutes for the mother’s post-natal check. Further details about what happens at this appointment is available from NHS choices.

Please remember to bring the red book to your appointment.

If you cannot attend this appointment you must notify reception as soon as possible.

Named GP

From 1st April 2015 all practices in England are required under the General Medical Services (GMS) contract to allocate a named accountable GP to all patients, including children.

Prior to the contract change, all patients at Warwick Gates Family Health Centre were registered with a partner and had a ‘usual’ GP allocated, on a pro rata basis. This was an administrative exercise to allow for the equitable allocation of test results and letters that come into the practice. For the purposes of this contract change your ‘usual’ GP will become your named accountable GP.

The contract remains ‘practice based’, so overall responsibility for patient care and appointment booking systems have not changed. This is largely a role of oversight, with the requirements being introduced to reassure patients that there is a GP within the practice who is responsible for co-ordinating their care as required.

There is no requirement within the contract for patients to see their named GP and patients are free to see any GP at the practice, as they currently do.

What are the responsibilities of the named accountable GP?

The named accountable GP will take responsibility for the co-ordination of all appropriate services and ensure that they are delivered to each of their patients where required (based on the GP’s clinical judgment) to each of their patients.

If you would like to know who your named GP is, please ask when you next attend the surgery.

Please note that whenever a GP leaves or joins the partnership, the health authority reallocates the patient lists. It may be that your named GP will change under such circumstances. We are not able to contact patients individually about such changes, but we will publicise any partnership changes and advise patients wishing to know who their named GP is to ask at the surgery.

Access to Medical Records

In accordance with the Data Protection Act 2018, General Data Protection Regulations and Access to Health Records Act, patients may request to see their medical records. There is no charge for this as long as the request for information is reasonable.

Please speak to administration team if you require a copy of your medical records.

No information will be released without written patient consent unless we are legally obliged to do so.
